Follow the process
Rather than read a checklist mechanically, select a process such as procurement, complaints, training or order fulfillment. Ask what inputs arrive, who owns decisions, how success is measured and what happens when a requirement is not met.
Separate observation from judgment
A good audit note identifies the requirement, objective evidence and consequence. Avoid opinions about individuals. Interview more than one shift or role when evidence varies. Keep records factual and proportionate to the finding.
Close the loop
Corrections address an immediate problem; corrective actions tackle the cause so the issue does not recur. Review implementation and effectiveness rather than marking an item closed solely because a form was signed.
Put this into practice
- Audit one handoff from request to completed service.
- Collect at least two forms of evidence for a significant conclusion.
- Check whether corrective actions changed the observed failure rate.
Frequently asked questions
Must every internal auditor be an external lead auditor?
Not necessarily. Organizations should ensure auditor competence and independence appropriate to the audit.
What makes a finding useful?
Traceable evidence, a clear requirement and a practical path for correction and follow-up.
